Okay so I have to do this survey for an English Course and I hope a lot of people would answer.

0. How old are you and where do you live (Country)?
1. Select the best fitting option from following:
A) My sleeping habits are excellent and I feel well rested (almost) every day.
B) My sleeping habits are very good and but I feel tired from time to time.
C) My sleeping habits are not so good but I can manage with the amount of sleep I get.
D) My sleeping habits are good but I often feel tired.
E) My sleeping habits are bad and I often feel tired.
F) None of the options above.
2. Select the best fitting option from following:
A) I know there is some factor that effects my sleep negatively, which? (Can be diet, alcohol, smoking, activities, stress...)
B) I do not know any factors that effect my sleep negatively.

A) I have one or more things that I know helps me sleep better, which? (Can be diet, activities...)
B) I don't have any things that help me sleeping.
