Floating Head above Corpse

I have seen this problem so many times with my models, mostly before I joined the online moding community. I usually just delete the model.. But now that I have finally made an account here I have asked for help.

I take a Knight, for example, put a captain's head on it, and as you may know there is a head on two geosets so I must replace both heads. This is what happens..

War3 Model Editor:

Stand and Attack Animations.. Head #2, I will call it, shows under Head #1 and is not moving because the bone it is attached to is not supposed to move during this animation. When I go to the death animation Head #2 is working properly but Head #1 still shows.

In-Game:

When the unit dies Head #1 floats above the corpse.

I apologize for this is poorly explained and ask questions if you wish..

Can somebody tell me how to fix this.. But I don't want to know how to fix it only on a Knight.. On all models..
 
Also I am using Vertex Modifier. That is just what the end result looks like in the Model Editor..

I have another question.. Today I was making "WoW Style" Voidwalker and I was looking to add shoulders using the Geoset Merger but I didn't know which bone to attach it too.. How can I find out.. Most of the bones were like Mesh_23: Mesh_31, Object08: Mesh_12, etc...

I am new to modeling so I need some help with it.
