A few notes:
Don't talk about triggers if the map is protected.
Don't use ratings, go by instinct: using "10 points for originality, 10 for terrain" etc is far worse. In some games terrain isn't very important, in others it is - you can't put a static score on that.
Also: because of those points, some maps that should get rejected get approved and others that should get approved get rejected - I highly advice you to drop that template of yours.
Originality is a rule. Although it can be a positive thing for a map, it isn't supposed to be graded separately.
You often give full points even though you've got a negative remark (?)
