Basically to use an item as an attachment by picking up an item you need to attach an ability to the item that uses the model as an attachment. Each item can have multiple abilities.
Make a custom attachment ability where you set the 'Art - Target' field to the attachment you want (your model) and the 'Art - Target Attachment Point' field to where you want it attached (hand, left) (hand, right) etc.
Now just add this ability you made to the item's 'Abilities' field.

The way the item looks ingame (when it is sitting on the ground) is determined by the model file of the item in the object editor. Most of them are the treasure chest model by default, but can be changed to whatever you want.

When you open the field Art - Target Attachment Point 1 there is a button that says add string. For the first one write 'hand'. Then add another string and write 'left'

Leave out the quotes when you write the string though.

You probably know the items Orb of Fire, Venom, Ice etc. and all these items create a special effect on the weapon of the hero wielding it, so what you can now do is customizing an item for example like orb of fire that it creates a specialeffect for example at the "hand, right" of the hero wielding it.
