special effects problem

i have a special effect so when a hero gets and item it adds a sword to his left hand. now on someheros, it goes to his left hand, but on others it goes to their left foot?

i tried left weapon also and got the same thing... how can i make it always in their hand?

for the heros that were on their foot, are they

mounted heroes(horses etc.)
alot larger heroes?

those are the only two problems i would guess but ive never had this problem so i wouldnt know 100%
