Download the Portrait_Icon.mdx model attached below and open it with MDX Pather.

You will see a window that looks like this:
MDXPather.jpg

Replace the existing icon path with the path of your icon (if it's a custom icon, the custom icon has to already be imported into the map with that path in the Import Manager).

For example change this:
ReplaceableTextures\CommandButtons\BTNMurlocMutant.blp

to this:
ReplaceableTextures\CommandButtons\BTNJaina.blp

Save the changes; close MDX Pather and import the Portrait_Icon.mdx into your map.

Change the path in the Import Manager from "war3mapImported\Portrait_Icon.mdx" to the path of the unit model and add "_Portrait" at the end of the name, before the ".mdx" file extension.

For example if the path of the unit model is:
Jaina.mdx

then the portrait should have this path:
Jaina_Portrait.mdx

Change this (in the Import Manager):
BTNJainaWhite.blp
and this (in the MDX Pather):
C:\Users\Xaviere\Downloads\Proudmoore.blp\assets\BTNJainaWhite\JainaWhite...
to this (they need to be identical):
ReplaceableTextures\CommandButtons\BTNJainaWhite.blp

I recommend reading this if you are still confused.
