How do you add models onto a unit when they get an item?

I have lots of maps and I don't do much on Hive but I'm confused about how you collect an item then a model appears on the unit you gave it to. Say for example if a Villager (Male) picked up an item, he whould suddenly be wearing a helmet. Please answer my question.
