Will expose what happens to the hero with some amounts of HP:
With 2350 hp:
When you are on 500 HP, your bonus regen will be bonus 9.35 hp per sec
When fall to or under 235 (10%) you get 23.5 hp reg per sec for last 5 secs. Thats meen 117.5 for 5 sec. Than you are on ~350 hp. the bonus reg you get for theese are ~10. When your hp rise up, the bonus reg you get will be reduced.
With 5000 hp:
When you are on 1000 hp the bonus reg you get will be 20 hp per sec.
When hp is 500 or lower or have 50 hp per sec reg for 5 secs. Thats meen you will be on ~750 hp. Ypur reg bonus on theese hp will be ~21 per sec)

When you are on 5000hp you have 100 reg permament- each time, but in battle.
HoT dont helps you in battle. You can get regen after 6 long secs. Thats can be crucial!
With HoF you get regen and IN battle where you actually needs reg. When you looses hp that increases your reg, thats nice.
