Intermittent Desync When Creep/Boss Dies

I can't figure out why but there is a desync that only happens sometimes when the Fire Golem boss/creep dies in my map. I've read through this post and removed a lot of triggers. But this desync is still lurking. Does anyone have any suggestions? I'm totally puzzled. Thanks for any suggestions.

Just one that displays a text message and plays a sound for all players.
