The below doesn't appear to be working for me either. Only the extended tooltip gets changed.

  • Untitled Trigger 001
    • Events
      • Time - Elapsed game time is 5.00 seconds
    • Conditions
    • Actions
      • Set VariableSet TempItem = Claws of Attack +15 0001 <gen>
      • Game - Display to (All players) for 30.00 seconds the text: ((Name of TempItem) + has been modified.)
      • Item - Set Name of TempItem to Test 1
      • Item - Set Tooltip of TempItem to Test 2
      • Item - Set Tooltip of TempItem to Test 3
      • Item - Set Extended Tooltip of TempItem to Test 4
I'm not sure this helps, but it may help demonstrate your problem to others. Can anyone else comment if this is user error or a bug?

Edit: Fixed up an error in the trigger and attached screnshots :) Results seem the same regardless of event and only affects the Extended Tooltip. Attached pictures for reference

I actually have an inventory and item system that stores all it's effects and attributes as a string in the icon path of the item. It's name is always randomly generated like in Diablo based on what stats it has and it's rarity. So while your suggestion would probably work in most cases, my system creates so many unique items that it would be unreasonable for me to use that solution.
