Demon Knight, but I just removed the eyes.
Oh, and this is 90% by me (I think) because teeth were recolor and smudge. And I used no filters.

It's still not exactly 90% by you as you used the skeleton mage icon as a base to trace from. It's a pretty good likeness of the model tho, you should try edit the icon to feature the same eyes

Add a sharpen filter (the only good filter) on the 64x64 image (excluding the borders), and then I'll approve it.
