When I think of this icon without the name reference, I tend to think of it more as a "Energy Drain" than "Breath of Fire".
Fire is expanding and has smoother lines, while energy is concentrated and has crisp movement.
Some little things, like a spark here or there, or making the breath bigger further away from the peasant is, or some smoke coming up, would greatly enhance the fire feeling of the icon.
