Adding effect to my spell.

I have a item that uses the ability "disease" to damage nearby units. But I'm trying to add an effect to the victims while being attacked. I want to use shadow strike. But I cant get the effect to show up.

Not sure what you want. If you want to add a missile, then it is more complicated.
If you just want to change the buff model, you have to change Art - Target.

I see that, you are using the buff from the tornado slow aura. Be aware, that for the owner of the aura the buff model is not displayed, if using aura abilities.
You would have to change the model of the aura in the ability.

Do you want the ticking damage numbers? That would require some dummy units casting a version of shadow strike. Or a damage engine for you to create the floating text yourself.

I want to have the disease effect above the workers while they're being affected. Does that require missle?

Just set the Art-Target to Shadow Strike <Target> and Art - Target Attachment Point 1 to overhead
