[Suggestion/Request] Description style suggestion for an Altered Melee

I'm working on my map and I need help with the Tooltips
I know how to work on it but I don't know a good format (and no I DON'T want to use the vanilla version of the tooltip format)
Can someone give me a good tooltip format?

@OP, Tooltip formats usually depend on the spell itself. Personally, this is the format I prefer (scroll a little below)

Not when you're learning it. It may be 100 now, but when you upgrade it, the cost may increase.
