Simple add texture to model.

I will like to make a very simple model request considering that i don't have any knowledge with creating models I have come to ask.
All I need is a model to get this persific texture working as it doesn't seem to work with using a replaceable texture.

sorry I forgot to add the texture string to my post as i was in a bit of a rush :/.
All I want it to get a simple 2D square model with it showing this texture.

Also does it have the same kind of transparency like it is in in the menu in game?
This is for a Hero choose UI system and this is the last thing I need for it and want it to blend in with other dialogs.

I need this model to be transparent like in the menu/dialog when playing Night Elf.
I am in big need of this and I don't have any experience in modeling myself to do it.:/
All that I am asking for really is to make a texture into a 3d model with simple transparency.

yeh but still less transparent.
go onto Wc3 and play a map as Night Elf and go into menu and the menu has the texture. I want the texture to be as transparent as that.
