Need Help Importing A Custom Model!

This is my first time trying to import custom models... ive followed directions from multiple tutorials but i cant get it to show up.... ive attached my map and the custom model stuff. i want to change the model file of the custom hero Exiler Seer to the custom model. Im sure you smart ppl could quickly see what im doing wrong. ..please help....btw it just shows a green checkered box
