Need Help Adding a Very Simple Death Animation

Am trying to add a very simple death animation to this model. I need it as a projectile and a simple alpha 1 to 0 disappear will do. The problem is whenever I try to make the change myself my game always crashes when I use the model, so I did something wrong. All I need is a simple death animation that will make the model disappear. Does anyone know how to do this?
