Loading screen Basic

Hey im requesting some 1 to make a new loading screen from the image below

what i need from you is to add a transparent(so u can still see the image kinda) box in the center with some kind of fancy boarder.
also above the box the title of my map witch is "Footmen Vs Grunts Elite" preferably a gold colour or silver im not too sure what would look better
leave the box blank and ill add the info about my map in there my self
also leave a highlighted space near the title where i can add/change version numbers as the map progresses
