Is it possible to have it so that when your hero picks up an item say a sword for example it shows a sword in their hand?

Yes. You can make a skill based off of something like Sphere or Tornado Slow Aura that will just add a special effect to a point on a unit (like "weapon" or "hand, left") and give that ability to an item.
