to change the map name simply open your map and hit scenario then in the drop menu hit map description then put in the title.
for the loading screen goto the same drop menu but hit loading screen, to use a custom picture in the background of this it is advised to get MagosX.com it is a program that allows you to reshape the picture and save it into the right kind of files
After you have you picture shaped import it using warcraft3's import manager(right next to the test map button.) you must import a copy of the picture as .mdx and the othe as .blp.
Then goto the customize loading screen menu and hit the use imported loading screen bubble. Make sure you use the .mdx for the actual file for this not the .blp.
Other useful information to have a nice loading screen
This is a quick guide on how to change the text in the loading bar. By default its set as "L O A D I N G", "Press any key to continue" and finally "Waiting for other players". Because the map has to load before these changes can be made, the L O A D I N G will not change until players play another game.
1. Open notepad, or any other word processing program.
2. Copy and paste the following:

LOADING_PRESS_A_KEY=Text Here
LOADING_WAITING_FOR_PLAYERS=Text Here
3. Replace "Text Here" with your own text. By default, the text is centered.
4. Save the file as "war3mapSkin.txt". (Note the capital S - it's important)
5. Import to your map and make the custom file path "war3mapSkin.txt".
6. Enjoy!
