First good model
Second yes you can attach it ti the hero that acquirs the item you want.
You can go to the spell of the item and somewhere says caster, it wants a model, you put the belt model there or whatever model you need. Then a slot down it asks the position of the attachement. You can put head, arm left, arm right, overhead and others.
