Is there a way to make an ability totally from scratch or do you have to base it off of an existing ability and change the look and sounds and stuff?

Using the Object Editor, you must base it off an existing standard (non-custom) spell. I am not sure if there are any other ways but advanced spells that create some fancy pattern use triggers.
