Use a DDS (Damage detection system) found in the spells section.
Then using the event that is described in the DDS check if the unit being damaged has the item. Then in the actions does what you want.
You attach a unit is damaged trigger event to a trigger when a unit acquires the item. You then recreate the trigger when the unit loses the item. Care must be taken to only destroy the trigger in a way that will not crash the game (not sure how).
