Looks good if it is to used as a small creep or critter. If you want to use in a normal scale then it needs work! I will give you a 4/5 for a good attempt but an imperfect model!

You should make a head model and then body models so people can make growing worms.

I agree with Donut. Really good point! You can also make this thing in other colors or other style like it is more cylinder like or something like this! I still like it but it can be better in many ways!
