[HD/Modeling] Add Dissipate Animation to Model

I have been trying to figure out how to add a Dissipate animation to this model (the first one listed: Forsaken_Apothecary.mdx) and keep running into issues. I was wondering if someone with more skill could possibly add it for me. Thank you in advance for any help!
