It's done by creating modelName_portrait.mdl by putting 1 texture with picture at front (where the camera looks)
Can you create a model like that for me? I only know how to change textures. As for camera do I use camera object?

Cameras are a type of node inside models.
I normally use camera manager and create new from current view (rename to 'camera') . That way you no need portrait models.

Btw do you know how to create a model like anufis described? I'm bad at modelling.

You just have to open a .blp model and save it as mdl or mdx. Bam, you're done! It will just be a plane with your texture applied on it, then you can create a camera and voilà.

In the import manager, just make sure to rename it as "YourHeroName_Portrait.mdx"
