I'll put a massive!!! amount of ideas. :D
"Wand of the Old Gods"
Description : This wand was created using the very essence of C'thun and Yogg-Saron. The wand has a fragment of their powers.

Bonus(es) : +150 Mana
+15% Mana Regeneration

Level Requirement? : None

Second - "Staff of Velen"

Description : The legendary staff of Velen, the Draenei prophet, gives the user great intelligence and wisdom.

Bonus(es) : +20 Intelligence
+150 Mana

Level Requirement? : None

Third - "Succubus Wings"

Description : Wings made from hard metal to represent Succubus wings.They won't make you fly.

Bonus(es) : +30 Agility
+60 Movement Speed

Level Requirement? : 3

Fourth - "Boots of the Planes Walker"

Descriptoin : Yuk'Ne'Velr the Planes Walker used to wear these boots. The legends say they allow the user to blink.

Bonus(es) : +400 Range Blink(Spell)

Fifth - "Rusty Buckler"

Description : A rusty buckler that have seen many battles.

Bonus(es) : +5 Armor

The Mookreaver:

A gigantic warhammer that is more powerful when facing multiple weak units, but loses power when against single/strong opponents.

If there are no enemies around:
- +10 damage
- (Chance of) knockback

Whenever weak enemies are in range (weak being, say, less than 1500 hp):
- Gains one damage per enemy in range
- Does splash damage
- Gets a new passive AoE ability for every five enemies present (cleaving attack, pulverize, AoE knockback, etc.)
- Increase attack and move speed by for every 10 enemies in range

Whenever strong enemies/ heroes are in range:
- Loses 1 damage for every 100 hp over 1500
- Lower attack and move speed for every 200 hp over 1500
- Loses AoE knockback, loses knockback if enemy is VERY powerful (bosses etc.)


Change the numbers for balancing.

Illuminara:
The sword of Dardrin the Wise, it was crafted out of culminar by the finest Dwarven smiths, enchanted by the greatest Elven magicians, and blessed by the greatest Human Saints.

Effects:

+XX Damage
Dispels any illusions and confusion on wielder and allies.
Can inflict Insight on an enemy, stunning the enemy and causing it to look over its actions. If the victim is corrupt or evil, it takes XX damage and is weakened. While stunned the target cannot be auto attacked. Attacking will break the stun.
Can cast Illuminate, which entraps target foe in a pillar of light, effectively removing it from combat doe XX seconds. Also increases ally attack, defense, speed, health, and mana by 5%.

Disdrst:
Illuminara's twin, it was shattered during the Daemon Wars. Part of the recovered shards were then reforged into a dagger. Though the dagger has none of Disdrst's former powers, it is capable of new powers.

Effects:

Absorbs a portion of all magic damage taken and channels it into the next physical attack.
When the wielders mana is maximum, Disdrst siphons off the wielders mana into the pommel, storing it as a mana source.

Dancing Blade

This finely balanced blade seems to contain a life of its own...

* (small) Agility bonus per enemy killed (and while equipped)
* Evasion
* Critical Strike

* Use:
Within this blade a demon is trapped and bound. When called upon, it will unleash its power in a single hateful strike against an enemy, dealing additional damage and stunning. However, the demon also lashes out against its wielder, stunning it.

(Wondering if maybe one could work in a use-too-many-times-and-the-demon-breaks-free mechanic, in which the item disappears and the demon is spawned, attacking any units in reach.)

I had some...

Sword of revealing light :
Deals +20 light damages, heal 5 HP per seconds, and deal 200% damages to undead units.

The Enchanted Armor :
Add +10 armor, reduce damage taken by undead units by 50%.

True Stealth Dagger :
Make the hero invisible for 40 seconds, if the hero attack, cast spells, channeling, etc, it'll break the invisibility, and it'll take more damages to attacked unit (just like wind walk)

Hammer of the Blacksmith :
Has a 20% chance to stun attacked unit, increases damages by 45, but it'll decrease attack speed and movement speed of the wearer by 50%.

Some Ideas...

How 'bout...

Calamity's Limit
- 5% chance to instantly kill the victim (does not affect bosses)
- Gives "Calamity" buff when activated (100% extra attack speed, magic immunity, but -50 hp/s regen for 15 seconds)

Description: A malignant god's ancient axe, lost in a world-rending battle between mortals and immortals. It can instantly smite its enemies with one blow, and it bestows its former owner's fell influence on its wielder, at the cost of their vitality.


Paradisal Opus
- Gives a 1% hp/s regen aura in 300 range.
- Heals target for 20% total hp (120 second cooldown, 200 mana)
- Grants +200 mana.

Description: A mysterious tome, penned by an obscure and insane priest. Its presence gives hope to those around it, and the holder may evoke the mystical healing properties the book possesses to aid their cause.


Sylvan Phylactery
- Allows the user to summon two wolves (you choose stats)
- Gives the user X agility and X hp.
- Allows the user to become invisible at night (Night Elf Shadowmeld)

Description: An amulet found deep in Silverpine Forest in a wolves' den. The holder may call upon the spirits of the forest wolves to help them triumph over their enemies. It also seems to impart energy and vigour to its owner.


Shade's Visage
- Gives permanent invisibility with 2 second fade time after casting/attacking.
- Gives True Sight.
- Can be activated to cast "Terror" (all nearby units run away and lose X mana/second)

Description: A shade's ghostly image, captured in a mask by a demented necromancer. It allows the wearer to fade from sight and see beyond what others see. When shown to the world, all enemies of the wearer will scamper for cover and their minds will be frayed by the horrible sight.


Orb of the Depths
- Gives +X damage.
- Gives a 10% chance to do 100% splash damage in 300 range (slows affected units by X% for 5 seconds)
- Can be activated to summon an Enraged Water Elemental (you choose stats)

Description: A mysterious orb found on an altar in a sunken temple. It spontaneously releases the hidden energy it stored while on the altar, creating a powerful water spout which injures and drags down the inflicted.


Gaia Breaker
- Gives a 10% chance to slam the area around the user (deals X damage and stuns for X seconds)
- Deals 30% more damage to rock/earthen enemies.
- Gives X strength.

Description: A sword known to be able to split boulders in two with a single strike. Its power resonates through the user, granting them superior strength and the ability to sunder the earth beneath them.


Storm Sceptre
- Grants the user +50% mana regeneration.
- Can cast "Tornado" (holds target in the air)
- Gives 10% chance to cast chain lightning on attack (X damage, X targets)

Description: The mighty sceptre of an archaic aeromancer (made up word). It contains the power of a raging storm, and has been electrically charged, so it randomly releases bursts of lightning.


Pontiff's Mitre
- Boosts the effect of healing spells by 15%.
- Grants X intelligence.
- Casts "Healing Nova" (heals all friendlies within a 500 radius)

Description: An archbishop's treasured headpiece, lost when his cathedral was raided by orcs. It is said to be responsible for the priest's superior healing powers and wisdom.


Hirsute Tunic
- Gives X armour.
- Grants 10% passive damage reflection.
- Grants +8 hp/s regen.

Description: A hair-covered tunic that provides moderate protection for the wearer. The bristle-like hair on this piece of clothing injures those who strike it, and it somehow invigorates the wearer.


Quisling's Dirk
- Gives X damage.
- Gives 40% extra attack speed.
- Gives 20% chance for a 3x damage critical strike.
- Gives X% extra damage against humanoids.

Description: The blade of a hermit, used to curb his own envy of the great hero, Pendragon (made up name). The hero's blood still stains the blade of this dagger. It seems weightless, and appears to find its victim's weak spots on its own. The blade was designed to pierce human flesh and stab through bone.


Vespertine Veil
- Gives 30% evasion.
- Gives X agility.
- Allows the use of "Ethereal Form" (only affected by magic and spells, can only cast spells)

Description: A dark veil that appears to absorb light and the silk-like folds seem to be extremely slippery. It was reportedly responsible for the disappearance of a young child, who enveloped himself in the cloak due to curiosity. He was never seen again.



Might add more, now that I've discovered HIDDEN tags :D. If the descriptions are too long, then you can shorten them, or even omit them altogether.

These are all products of my own imagination.
