You attached the sword to the wrong bone. Try attaching it to the "sword" bone.

You can just merge geosets and try to make it for the beginning.

You can also download the attachment 'simple cape' and see if it attaches to the chest without problems if you don't want to merge geosets (Geoset manager-> import/export in Magos).
