Custom Icons and Loading Screen Law

Is it okay to take a picture or an image from another game or website(as long as they are allowed to be used by that certain game or website) and use it as an icon or loading screen in my map? Should I put the name of the owner first? Or should the custom icon or loading screen be totally produced by me?

nope, it's okay, if there exist the owner, you should. not porn though. but if you are about to upload the icon into the icon section, it's not allowed.

and it's even better if you produce it by yourself.

It's okay, until you respect their rights and the restrictions they have given you.

If they didn't asked you to put credits, you don't have to, but doing it is kinda a way of retribution for the contribution, so it's better to do it.

One of my maps got rejected because of custom icons so I decided to remove it to get it approved. I actually put a statement that says I do not own the icons. The sequel of that map is on progress and I was thinking of adding custom icons and a loading screen to it. Hope it gets approved this time.

There's no reasons that it shouldn't be approved cause of icons, except if they are bad imported or ripped (without autorizations) from another game.

Or, If you did not state the credits. Credits are a must when uploading maps on THW.
