For that you'll need a few things:

1. A thread with an informative title. Something a bit more specific than "halpf".

2. To solve the actual problem, you'll need a periodic trigger with a timer, and a group variable containing all the ghouls you created. Create a timer, and set it to 20 seconds so that every 20 seconds you run a ghoul creating trigger.

3. Inside the ghoul creating trigger, first check if your ghoul group already has 20 ghouls. If it does, do nothing, otherwise create a ghoul and add it to the ghoul group.

Basically what Nowow said, although it's not necessary to use a unit group when you can check the number of living Ghouls owned Player X equal to Y. But if you want to use a unit group then make sure you remove the ghoul from it after he dies.
